A completed Physical Evaluation form must be delivered to your athlete’s head coach. Your athlete can bring this form with them on the first day of school to hand deliver to their head coach -- or you can drop it off at school at the main office. A student will not be allowed to practice until a Physical Evaluation form is in the coach’s possession.
- Transportation:
Jones Magnet will work with you to resolve transportation issues, but it is useful to have a direct number to the transportation office.
