Rawlinson Middle 2011 NCLB Report Card
Attention Rawlinson 6th Grade Parents:
Before 7th grade your student will require 3 new shots…they may already have them but the school will not have an updated record unless you brought or faxed the new immunization record to Rawlinson Middle School.
New shots required for entry into 7th grade are:
1. Tetanus booster, within the last 5 years
2. Meningitis
3. TWO total chicken pox/varicella shots… or documentation of the month and year of actual chicken pox illness.
Please take care of the immunizations as soon as possible to avoid long lines at the beginning of the school year in the fall. Please, fax ((210) 697-4055) or have your student bring the immunization record to the nurse as soon as possible.
