Medical Statement for Children with Special Dietary Needs
Texas Department of Agriculture -Child Nutrition Guidelines

"Each special dietary request must be supported by a statement that explains the food substitution that is requested. It must be signed by a recognized medical authority. In these cases, recognized medical authorities may include physicians, physician assistants, or advance practice nurse (Nurse Practioners).

The medical statement must include:
*an identification of the medical or other special dietary condition that restricts the child's diet;
*the food or foods to be omitted from the child's diet; and the food or choice of foods to be substituted.

The medical statement will be kept on file during the time frame the student is in school in the cafeteria and at the main food service office. A medical statement is required for special diet request and must be updated when moving from elementary school to middle school and middle school to high school if the school cafeteria is to assist with special diets."

Please contact your cafeteria manager if you need assistance with special dietary needs. We can work to make limited substitutions with other food items we have on hand to assist special diet requests. Parents will need to mark menus for their children with special dietary needs. The cafeteria managers will follow these during the year.

NUTRITION RULES FOR SCHOOLS

This year the Texas Department of Agriculture put new policies in place to limit snack foods in elementary schools. The policy states "An elementary school may not serve or provide access for students to Foods of Minimal Nutritional Value any time, anywhere on school premises during the school day."

Specifically, elementary students may not have carbonated drinks at school during the school hours. Also banned are most kinds of candy, gum and popsicles (unless containing fruit juice or milk). This will not affect activities after the school day or on weekends. Also not affected are individual meals sent from home to be consumed by the student and not shared with others.

For more information on the new policy and suggestions for nutritious snacks, please see the links below.

Elementary Schools Only

Peanut Butter Sandwich & Milk
Read about the Northside ISD Cafeteria Policy

It is Northside's intent that children not be hungry at school; however, it is the responsibility of parents or guardians to pay for meals provided, or to provide breakfast and lunch from home. At the elementary level, your child will only be allowed a maximum of 5 meals in cafeteria charges.

After your child has accrued 5 meals in charges, your child will receive an alternate meal consisting of a peanut butter sandwich and a milk.  This alternate meal will cost 60 cents.  Students will continue to receive this alternate meal until all cafeteria charges are paid.  Proper notification will be made to persons responsible for payment.

Please notify your school's cafeteria manager if your child is allergic to peanut butter or milk so another alternate can be provided.

Remember that Free & Reduced Meal applications are available throughout the school year by calling (210) 397-4517 or at the school offices.  The Free & Reduced office hours are Mon - Fri 7:00 a.m. to 3:30 p.m.

Checks for payment on a student's lunch account will be accepted unless there is an NSF check on file. EFFECTIVE JANUARY 1, 2006, THE NEW NSF FEE WILL BE $25.00. The cafeteria manager will discontinue accepting checks from the individual for the remainder of the school year on all accounts. Only cash, money order, or cashier's check will be accepted for repayment of the returned check and further payment of meals. A letter will be sent from the Child Nutrition Office to notify individuals that the cafeteria can no longer accept checks.
