Parents may register students for 2012-2013 during office hours till June 18. The office will reopen on July 30, 2012, at which time,
registration will resume.

Note: During the summer, the office will close at 11:30 AM on Fridays.

Out of District Student

• Certified birth certificate
• Child's social security card
• Immunization records

Kindergarten- Children may begin Kindergarten if they are 5 years old as of September 1st.

Pre-Kindergarten- In order to be eligible for the Pre-Kindergarten Program, your child must be:

• 4 years old on or before September 1st AND
• Limited English Proficient as determined by the Home Language Survey and the Bilingual Syntax Measure OR come from a family whose income, according to standards set by the State Board of Education, is at or below subsistence level (based upon the free/reduced lunch income guidelines).

• If you are unable to register your child on July 12 from 8:00 - 11:00, you can register him/her at Colonies North beginning August 1, 2012.

Documentation required for registering for Pre-Kindergarten

• Certified birth certificate

• Proof of income (current paycheck stub(s)- 1 if paid monthly, 2 if paid biweekly, 4 if paid weekly

Those families on food stamps must bring the Notice of Eligibility; families on TANF must bring the Benefit Letter from the welfare agency. Those families who have received a Predetermined Notice of Eligibility for the lunch program from the Northside Child Nutrition Services department must bring the letter at registration.

Immunizations: An updated immunization record that includes the following immunizations:

• DTP/Td/DtaP - 5 doses (one dose must be on or after the 4th birthday)
• Polio - 4 doses (one dose must be on or after the 4th birthday)
• Measles - 2 doses
• Mumps - 2 doses
• Rubella - 2 doses
• HIB - 4 doses (not required after 5 years of age)
• Hep B - 3 doses
• Hep A - 2 doses (2nd dose must be given 6 months after the first dose)
• Varicella (chicken pox) - 2 doses or verification of illness
